About this guide
An independent guide to Carson Smith Opportunity Scholarship (CSOS) Program, not an official application or an eligibility decision. Rules, application windows and available funds can change. Confirm your circumstances with the program before changing enrollment or committing money.
Before you start
Check CFEF's provider and service requirements
Review the provider application and current fund-use guide. Relevant business identity, professional qualifications and any required insurance or licensing must support the services you propose.
Your next steps
Complete the CFEF provider application
Use the official become-a-provider page. Prepare legal name, contact details, business type, tax documentation and the experience or service information requested by the form.
Confirm approval for the intended service
Respond to CFEF's review and verify any category-specific requirements. Describe the educational service clearly, especially where the fund-use guide requires preapproval or additional supporting records.
Follow the authorized payment process
Retain the participation decision and CFEF's invoicing instructions. Coordinate approved services with the awarded family and provide itemized records without assuming every charge is allowed under the scholarship.
Preparation checklist
Use this to get organized. The current official application or handbook controls the full requirements.
- Legal business identity, tax documents and authorized contact
- Service descriptions and applicable licenses or insurance evidence
- CFEF approval and authorized invoicing instructions
Submit identity, financial, student and medical records only through the verified official process. Do not upload application documents to ParentSurf or include them in a public review.
Important distinctions
- UFA marketplace approval is not CSOS provider approval.
- Check expense-specific preapproval rules before delivering a service that a family expects the scholarship to pay.
