Before you start
Identify the student's eligibility category
Arizona's eligibility page is the starting point for age, residency and qualifying-category requirements. Use the category that accurately describes your student. Disability-related documentation and funding calculations are not interchangeable with the universal pathway.
Your next steps
Create the parent account and application
Follow the Applicant Portal link from ADE's eligibility page. Enter the legal parent or guardian and student information, and submit the documents requested for the selected category. Save the application number for follow-up.
Read the decision and contract
Watch for ADE's eligibility decision and any request for missing documents. Review the parent handbook and contract before changing enrollment or committing to purchases. Approval, a signed contract and an available account balance are different milestones.
Activate the correct payment workflow
Enter ClassWallet through the ESA portal. Arizona explains marketplace purchases, direct payments and reimbursements separately. For reimbursements, keep a detailed invoice, payment evidence and any applicable provider credential required by ADE.

- Student and guardian identity and residency records
- Evidence requested for the chosen eligibility category
- A plan for education expenses and required supporting receipts
Submit identity, financial, student and medical records only through the verified official process. Do not upload application documents to ParentSurf or include them in a public review.
Important distinctions
- Do not withdraw from a school based only on an unsubmitted or pending application.
- A purchase being available online does not establish that Arizona ESA will pay for it.
